The URL you use is server specific, and it fails because of this reason alone. You need to make a more real test, sending the same request, with the same Host header. I.e. if you use a browser you need to use the exact same URL in the url-bar, and direct it to the right server by other means. Alternatively, if you only host a single site, you can configure Squid with the defaultsite= http_port option telling your Squids what the actual site name is (not the per-server IP, that should NEVER be specified here...).
