Any thoughts on where I can look to maybe get the ball rolling with
figuring out why this is happening? Thx

Somewhere that tells you the version of Squid you are using. We might ave better suggestions when we know what to pass your way. (If its 3.0 before STABLE5, there was a bug, upgrade).
