Are you doing transparent interception, or is the clients explicit
configured to use the proxy?

If transparent interception make sure you handle the related ICMP
traffic properly or you easily run into MTU problems if the clients have
a smaller MTU somewhere in the path...

It's very weird and so far I haven't found anything else to solve the mystery.

The customer told me their ISP replaced a bad 'something' (a fibre/ethernet converter maybe) just a couple of days before the problems appeared (or got noticed) and I asked them to check with their ISP, but -as usual- 'it's all OK on their part'.
