it very looks like maxclient issue on apache webservers , how can i
fix this issue ?
None can answer that without knowing what your config is doing.
Check for errors in cache.log, check your squid.conf http_port is set to
do interception properly.
If you still can't figure it out show us the config and a trace of the
problem transaction which involves headers.
