Yes, I spotted that after posting. However, removing the second reference did not remedy the situation. It seems that the server name is removed. For example, if the URL requested is "http://news.bbc.co.uk/2/hi/africa/default.stm" then squid appears to remove the primary DNS request (in this case "http://news.bbc.co.uk") and then generate the error based on the directory and file name of the requested URL (in this case "2/hi/africa/default.stm").
