You are missing the cache type value.  Your configuration line should read:

cache_dir ufs /usr/local/squid/var/cache 100 16 256

or:

cache_dir aufs /usr/local/squid/var/cache 100 16 256

or possibly:

cache_dir diskd /usr/local/squid/var/cache 100 16 256
