We run same config with Etch, Squid from Etch, but without Dans Guardian. Never had anything like that. I'd check you are clearing the cache completely (rm -rf is your friend - honest), and check Dans Guardian, and the Debian BTS if you haven't already. I think folk will need more config details to guess at a config mistake.
