Sory to come back with the same suggestion, but have you:
 - looked into cache_log? (particularily when squid starts).
 - looked at cache manager's 'Current squid configuration'? (and check
cache_mem value).
 - checked ulimits?  You may want to modify your squid rc.d script to
increase hard & soft ulimits.
