Fresh (updated) install of CentOS 4.3, Squid 2.6Stable3 using the
configuration parameters supplied above (plus ufs storage type),
completely stock squid.conf works fine:

I'm downloading the CentOS 3.8 DVD ISO right now to set up a test
machine. Testing will probably have to wait until tomorrow.
