Fresh (updated) install of CentOS 4.3, Squid 2.6Stable3 using the configuration parameters supplied above (plus ufs storage type), completely stock squid.conf works fine:
[root@vmwarelinuxtest squid]# uname -a
Linux vmwarelinuxtest 2.6.9-42.0.2.EL #1 Tue Aug 22 23:56:05 CDT 2006 i686 athlon i386 GNU/Linux
[root@vmwarelinuxtest squid]# sbin/squid -v
Squid Cache: Version 2.6.STABLE3
configure options: '--enable-epoll' '--disable-poll' '--disable-select' '--enable-async-io' '--with-pthreads' '--enable-storeio=coss,aufs' '--enable-large-cache-files' '--enable-snmp' '--enable-coss-aio-ops' '--with-large-files' '--disable-ident-lookups' '--prefix=/usr/local/squid'
[root@vmwarelinuxtest squid]# sbin/squid && head var/logs/cache.log

I'm downloading the CentOS 3.8 DVD ISO right now to set up a test machine. Testing will probably have to wait until tomorrow.
