Thanks Henrik, I've installed squid-2.5.STABLE12-18, which seems to log the user name correctly. If it misbehaves under "load" I'll try the latest version, instead of the handy RPM by the friendly folks at suse.

ps. I originally installed -3.0-beta because I could't work out why pam_auth simply did not work on squid-2.x. The problem was that suse's apparmor profile for squid refused to run external authenticators .. it was missing this from /etc/apparmor.d/usr.sbin.squid in opensuse 10.1 :

  capability dac_override,     # did I add this?
  /usr/bin/ntlm_auth rix,
  /usr/sbin/pam_auth rix,
