You have posted this squid.conf line:
external_acl_type ntlm_group ttl=0 concurrency=5 %LOGIN
/usr/lib/squid/wbinfo_group.pl
Why you are using the "concurrency" parameters ?
You must use "children" instead.
For details look into squid.conf.default.
