i've read through:

ViSolve's Squid 3.0 Configuration Manual
and,
searched the list archives ...
and all indications are that these directives should be ok.
have i missed a known issue, or am i missing something obvious in
the 'upgrade'?
Simply ViSolve's Squid 3.0 Configuration Manual is very outdated ....
Many Squid 3 squid.conf directive are changed, you should look into
squid.conf.default and create a NEW squid.conf.
Please don't forget that Squid 3.0 is still a DEVELOPMENT version,
without the code quality of 2.5.
