Is this possible? Squid keeps looking up the real www.example.com and

Squid version ?
Recent SQUID versions use /etc/hosts (too).

Sorry, my bad.

Squid 2.5 Stable 9 compiled with:
./configure --prefix=/usr/local --disable-ident-lookups --enable-ssl --with-openssl=/usr/local/ssl --enable-useragent-log --enable-referer-log --enable-storeio=ufs,null --disable-internal-dns

We also have hosts_file /etc/hosts uncommented in /etc/hosts.

AFAIK, the problem is it finds the real www.example.com in dns before it checks /etc/hosts and
we don't want it to look at dns at all, for anything.
