The NTLM layer (no caching):

no_cache deny QUERY
cache_dir null /null

The caching layer:
no_cache deny QUERY
cache_dir ufs /var/spool/squid 23000 16 256

no warnings in cache.log or from output of squid -k parse

full configs can be seen at:

http://www.nct.com.au/conf/squid.conf - bottom (caching) layer config
http://www.nct.com.au/conf/squidntlm.conf - top (ntlm) layer config
