Dave, As I recommend to just about everyone, try running dasdfmt and mke2fs manually. To do this, you'll have to start up rhsetup and get to the point where the DASD devices get defined. (The Red Hat installation process dynamically creates the entries in /dev after you tell it what volumes you're using.)
