macros naming conventions

Hello,

I'm trying to figure out macros naming conventions.
>From /usr/share/lib/rpm/macros:

 # Macros that begin with an underscore are "local" in the sense that
 # they (if used) will not be exported in rpm headers. Some macros that
 # don't start with an underscore (but look like they should) are
 # compatible with macros generated by rpm-2.5.x and will be made more
 # consistent in a future release.

I know it explains it there, but I'm not sure what "local" here
means. Also, there are macros that start with a double underscore, Are
they different from the rest?  For example: %buildroot, %_rpmdir, and
%__install, which are similar in that they all expand to a path, all
follow different naming schemes. Just curious:-)
