Does anyone have a KVM they like?

Yep, the Black Box R722 - 4 port - works with PS2 ports - very stable and handles video resolution up to 1600x1200. Cables are pricey and they recommend coax versions for long runs or high resolution (1280x1024 or greater) but they just work. They also make an 8 port version that is well regarded, but I have no direct experience with it.

Is anyone using a USB KVM successfully on Linux?

Nope and tried several
