My guess is that he means mutt doesn't use SMTP (AFAIK) to send mail to a remote MTA, but sends it to the local MTA.
Personally, I like mutt a lot, but I wish it had a nice address book integrated with it. If the mutt folks would merge the abook program or something similar into it, I think mutt would be more attractive.
