QLogic PCI to Fibre Channel Host Adapter for QLA2340:
Firmware version 3.03.11 IPX, Driver version 8.00.02p11-fo

The servers in question only have 1 HBA in. However /etc/hp_qla2x00.conf has failover and load balancing set ...
# cat /etc/hp_qla2x00.conf

load_balancing = 1

The servers will run fine for several weeks, then suddenly the error will occur.
