I just did an Oracle 10g Standard Edition install today. Non-RAC,
totally run off of a EVA5000 SAN to try to see what could be occurring.
I am not seeing any of the problems you are.... I am on HP FCA2214 HBA's
firmware rev 1.45.....

A real engineer only resorts to documentation when the keyboard dents on the forehead get too noticeable.

Now it's my turn for a late response.....
What kind of Fibrechannel HBA's are you using, and what driver version?
Are you using multipathing or card based failover mechanisms?
and in a similar vein, which HBA firmware versions (microcode) are you
using? (although I'm probably only any use with HP HBAs, I'd have a
look at upgrading them).
