We had a similar problem and it turned out to be the KVM itself. We had to run a mouse reset string on the KVM and all was well after that. I swapped out my Microsoft intellimouse for an optical mouse and kudzu sees the removal of the old mouse, but doesn't add the new mouse. The mouse works on all systems on my KVM but the RH9 box. I have put the Intellimouse back on the KVM and it works fine with every box but the RH9 box. I have tried editing /etc/sysconfig/hwconf and removing the mouse entry, but still kudzu doesn't see a mouse. What gives???
