This has crossed the list before and believe it or not I answered then

with the solution being to mount the iso to the cdrom as detailed here

I have also seen the same problem running under VMWare.  (Disc 2 would 
fail under VMWare, but would work fine while not under VMWare.  Never 
found out why.)
