check that /usr/share/squid/errors directory exists or not.
it might be there but there will be serveral sub directories for langauge selection:
 
make sure that "/usr/share/squid/errors/English/" directory is there and then define it in squid under the following tag:
 
errors_directory /usr/share/squid/errors/english
