I can't seem to get my printer to do anything less than about 3/8", but it may be a limitation of the printer. when I use: a2ps -R -B --lines-per-page=120 --borders=yes --margin=0 somefile I get a 3/8" offset, but the right border line is missing so my printer can't quite deal with it (or it is not tracking right, it's old). I use a HP lj6p. Does --list=defaults show the correct medium (A4, Letter...)? If you set --margin=40 you don't see a large left margin?
