Tcp_w causing problems just doesn't make any sense... Especially more so when NTP is
using UDP.
May not make much sense, but it does seem to be the problem according to your detective work. Check the man page carefully; you may discover that tcp_wrappers now has some impact on UDP as well. Also, check your firewall rules for things related to port 123 (any protocol, tcp or udp) both when it works and when it doesn't.
