This is Red Hat 4. We use IBM's sdd (Subsystem Device Driver) for device
multipath. Note the degrade state below:

# datapath query adapter

Active Adapters :2

Adpt# Name State Mode Select Errors Paths Active
   0 Host0Channel0 DEGRAD ACTIVE 1065540982 30 28 23
   1 Host1Channel0 DEGRAD ACTIVE 1021183874 30 28 22

and some SCSI devices' state is offline:

# find /sys/class/scsi_device/*/device/state -exec grep offline {} /dev/null \;
/sys/class/scsi_device/0:0:1:11/device/state:offline
/sys/class/scsi_device/0:0:1:12/device/state:offline
/sys/class/scsi_device/0:0:1:13/device/state:offline
/sys/class/scsi_device/1:0:0:10/device/state:offline
/sys/class/scsi_device/1:0:0:11/device/state:offline
/sys/class/scsi_device/1:0:0:12/device/state:offline
/sys/class/scsi_device/1:0:0:13/device/state:offline

Storage is provided by HP. Running commands `hp_rescan -a' followed by
`cfgcpath' made no difference.

Fortunately, the software running on the box, i.e. Oracle database, has
no problem. But I wonder if something is wrong and potentially can cause
issues. Thanks.
