I will second this - I've used qmail for many years, really like it, but it is getting more and more difficult to run on newer servers. Postfix is an excellent alternative and certainly a very secure/stable MTA - with all (most?) of the features you'll need/want out of the box. Qmail with extra features, even something basic like SMTP Auth, TLS, SMTPS will require patching.
