What I'd want and expect is a (per-soundcard) "master volume" that's persistent and affects ALL streams on that device, whether they're currently playing the moment I adjust the slider or not. I.e. the same functionality I'd get from a hardware knob on my speakers. I suspect this is the "reference volume" mentioned in this thread https://tango.0pointer.de/pipermail/pulseaudio-discuss/2009-May/003948.html (which also explains that the UI is mixing reference volume and virtual volume in a confusing way.)
