On a side note, the behavior of your NAT is also questionable (is it still Linux masquerade?) as it doesn't seem to do symmetric mapping for SIP traffic. This kinds of affirm our suspicion that Linux NAT (this indeed you're using Linux NAT) has some kind of SIP ALG inside.
