As I know PostNuke use ADO connection. Are the normal Mysql php function
faster than the ADO variant? How big is the difference?

ADO just wraps the base functions.

How is it possible to handle 40-50,000 visitors?

A second machine with Firebird database on ( Linux ), and keep Apache/PHP on the first machine. It will be 5-6 times faster, so you may still need to speed up on or other machine depending on where the bottleneck is.
