here is the code i wrote i didn't miss the $ sign and it works fine on web but returns the error message when i use it on localhost

Sounds like you left off a $ when using a variable. I would take a look at line 158, find the variable name and make sure it's $name. Disabling the error will cause the message to go away but not fix the problem.
