I do suspect you also have to open the database access first

$db = mysql_connect('YOUR_DB_ADDRESS','YOUR_DB_USER','YOUR_DB_PASS') or die("Database error");

mysql_select_db('YOUR_DB', $db);

I don't see where you set $username.



I would try

$qry="insert into videos(username, name,path) Values( $username', '$name',

'$path')";

echo $qry;

$insert=mysql_ query($qry) ;



That, and any mysql error message that you get, should show you how to

sort it out.
