I was able to reproduce your form  with a  dummy database 
that I made with the data you supplied in your code...


When doing this,  I noticed that your code you supplied  was 
missing a     right brace  }       you have   three left braces {
and only two right braces  }


also  there is no value assigned to   $ID     so the
ID=$ID  points to nothing (null)  when you click on the 
hyperlink of    Edit.php
