I currently have it like this:
<select name="BannerSize">
                                <option value="0">Select a Banner
Size</option>
                                <option value="728x90">728x90 -
Leaderboard</option>
                                <option value="160x600">160x600 -
Skyscraper</option>
                                <option value="300x250">300x250 -
Square</option>
                                <option value="88x31-300x250">88x31 and
300x250</option>
                                <option value="120x240">120x240</option>
                                <option value="940x30">940x30 - Pencil
Ad</option>
                            </select>
but your saying it should be <select name="BannerSize[]">
