For MySQL failures I use:

$result = mysql_query($query) or die(report($query,__LINE__ ,__FILE__));


function report($query, $line, $file)
	{
echo($query . '<br>' .$line . '<br/>' . $file . '<br/>' . mysql_error());
	}

Perhaps you can modify that for your use.
