Not to throw more questions into this book-stew - silly me - but I've
used Kodak.com for producing a calendar that was of quite good
quality and they also offer books - but they run up to $40 per book.
It is print on demand as near as I can tell.

Some information for the group regarding Lulu Press.
Here is a list of the graphics formats Lulu will accept:
CGM, G3Fax, GIF, IEF, JPEG, PNG, NAPLPS, BTF, Tiff, TIFF, Autocad
Drawings, FPX, SVF, BMP, XIFF, CMU Raster, Carnegie-Mellon Raster,
Portable Anymap, Portable Bitmap, Portable Graymap, Portable
Pixmap, Bitmap, Raw RGB image, Red-green-blue image, X Bitmap, X
Pixmap, SGML, XML.
Lulu Press converts everything to a PDF file. When the book is
printed, it is printed from a PDF file. It seems to me that if the
images look good in grayscale, they should print just fine. If the
developers are concerned about quality, they might upload some
sample pages with images of varying detail and order a copy of the
printed and bound book. You can create a book with a limited number
of pages. Or, after the manuscript is uploaded, carefully review
the returned PDF "proof."
The buyers on the list should also recognize that in some cases,
print quality can vary. So I am not sure everyone will receive the
same book. Just a guess; my experience with LP is limited to books
sans images.
I think some people are looking for some level of quality that Lulu
might not provide. Chances are, something will be lost because the
cost is quite low per copy. Now, if you want to offer a hand-bound,
hand gold leafed, embossed book with leather covers, hand marbled
endpapers, and 100% rag paper, ping me. The quality will be
fantastic, as will the price. OK, do not contact me, I offer no
such books on a large scale.
You get what you pay for.
If anyone past the developers of the book are interested, I suggest
visiting lulu dot com and learn about the process LP uses to create
inexpensive books.
