I understand how that could be done up to a point: indeed, another approach would be to never change the raw data of an image but simply store a script of how to get from what it was to how you wanted it. In PS as it stands not all actions can be scripted - I guess that for things like freehand brush stokes the length of the log file would be ridiculous: for some tools (airbrush) its not just the exact course of the tool but the speed it travels that affects the result. If you really really wanted to record everything you did in one session (including the undos) you could record the session as an action.
