The problems with backup someone else mentioned in this thread would remain, but you wouldn't have large blobs of data clobbering your "regular" queries. You could pass the scans and pics piecemeal between client and database without having to store them in the middleware (which may be an advantage or a disadvantage, mind you). Don't know whether PHP has bindings for that, though.
