Is this when you're replying to a post or creating a new post? If the former, and you're using reply-to-all, you'll be sending one message to the list and another directly to the poster of the message you're responding to. The directly sent message is outside of the list entirely, so any returned mail is also outside of the list. I've seen this happen occasionally myself. Could this be what you're seeing? AFAICT, such messages sent to the list *do* get filtered out.
