The path looks like it's a very unconventional installation. Usually, it should be something like /usr/local/... if it's on a unixoide system.
Maybe it's a Mac. This should be mentioned in the question.

We provide a PostgreSQL instance that we configure ourselves. An application is now needing the uuid-ossp extension which requires the –with-uuid=ossp flag added to the configuration.
So what happens when you execute create extension?
