What's the output of netstat while this is happening? Do you see connections to the remote MySQL server in netstat? What are the file permissions on pam_mysql.so? Does the access time of /etc/pam.d/pop (ls -lu or stat) change in accordance with attempts to login via pop3? What pop server are you using? What PAM service has your pop server registered itself as?
