Chris, actually I did read this. Is there supposed to be a config file in
/etc/pam.d for tally ? I have to admit that pam and I are not that familiar
with each other sigh!

I'll state the obvious.  Forgive me if you've already read this document...

/usr/share/doc/pam-0.75/txts/README.pam_tally, or something similar.
