What're major faults and minor faults?

Hi:

I was reading mm/memory.c the page faults management
part. Does every major fault corresponding to a disk
read? And a minor fault to only an allocation of page
frame?

by the comment in do_swap_page:
............
 /* Had to read the page from swap area: Major fault
*/
1145       ret = 2;
.............
but the line:
1131         page = read_swap_cache_async(entry);

may result in a cache hit, right?
