Set max_tgt to 1 in the host template field of your SCSI
host driver
and it shall scan only 1 target device. The error is as
there has
been no Start Stop Unit ready command performed on the disk
device.

Kindly send more information to be debugged like Version and
the
output from /proc/scsi/*
