+ proc-smaps-carefully-handle-migration-entries.patch added to -mm tree

The patch titled
     Subject: proc/smaps: carefully handle migration entries
has been added to the -mm tree.  Its filename is
     proc-smaps-carefully-handle-migration-entries.patch

The -mm tree is included into linux-next and is updated
there every 3-4 working days

Currently smaps reports migration entries as "swap", as result "swap" can
appears in shared mapping.

This patch converts migration entries into pages and handles them as usual.
