[merged] kfifo-fix-kfifo_init-in-include-linux-kfifoh.patch removed from -mm tree

The patch titled
     kfifo: fix KFIFO_INIT in include/linux/kfifo.h
has been removed from the -mm tree.  Its filename was
     kfifo-fix-kfifo_init-in-include-linux-kfifoh.patch

This patch was dropped because it was merged into mainline or a subsystem tree

The current -mm tree may be found at http://userweb.kernel.org/~akpm/mmotm/

include/linux/kfifo.h first defines and then undefines __kfifo_initializer
which is used by INIT_KFIFO (which is also a macro, so building a module
which uses INIT_KFIFO will fail).
