The patch titled
     gdth: unmap ccb_phys when scsi_add_host() fails in gdth_eisa_probe_one()
has been added to the -mm tree.  Its filename is
     gdth-unmap-ccb_phys-when-scsi_add_host-fails-in-gdth_eisa_probe_one.patch
