The patch titled
     vma_page_offset() has no callees: drop it
has been removed from the -mm tree.  Its filename was
     vma-page-offset-has-no-callees-drop-it.patch

This patch was dropped because it was merged into mainline or a subsystem tree

The current -mm tree may be found at http://userweb.kernel.org/~akpm/mmotm/

Hugh adds: vma_pagecache_offset() has a dangerously misleading name, since
it's using hugepage units: rename it to vma_hugecache_offset().
