The patch titled
     Add documentation for the vm_ops->access function.
has been removed from the -mm tree.  Its filename was
     access_process_vm-device-memory-infrastructure-fix.patch

This patch was dropped because it was folded into access_process_vm-device-memory-infrastructure.patch

The current -mm tree may be found at http://userweb.kernel.org/~akpm/mmotm/
