- megaraid-outb_p-extermination.patch removed from -mm tree

The patch titled
     megaraid: outb_p extermination
has been removed from the -mm tree.  Its filename was
     megaraid-outb_p-extermination.patch

This patch was dropped because it was merged into mainline or a subsystem tree

The current -mm tree may be found at http://userweb.kernel.org/~akpm/mmotm/

After conversations with the maintainers the _p isn't needed so kill it. 
That removes the last non ISA _p user from the SCSI layer to my knowledge.
